Empowering Tomorrow Foundation takes personal information and privacy seriously. As the surveys are meant to be anonymous, any identifiable information for you and/or your youth will not be attached in any way to the surveys.
Your/your child's information will be stored separately in a locked filing cabinet and/or encrypted file. The information will be used only for program improvement purposes. The surveys will not be shared outside of the study team, including with schools or other programs where your child may be enrolled.
However, there are unique circumstances that may require the research team to share your/your child's information with others: • If it is required by law due to mandated reporting of suspected child abuse or neglect. • If we think your child or someone else is harmed. • Sponsors, government agencies or research staff sometimes review consent forms and other study records. They do this to make sure the study is done safely and legally. Anyone who reviews study records is required to keep personal information private.
